The Employee Retirement Income Security Act of 1974 (ERISA) is a federal law that sets minimum standards for most voluntarily established retirement and health plans in private industry to provide protection for individuals in these plans.
ERISA requires plans to provide participants with plan information including important information about plan features and funding; provides fiduciary responsibilities for those who manage and control plan assets; requires plans to establish a grievance and appeals process for participants to get benefits from their plans; and gives participants the right to sue for benefits and breaches of fiduciary duty.

Electronic Delivery of Form 1095-C
We are providing this notice to alert you of an important change regarding the distribution of Form 1095-C. McCoy’s Building Supply is no longer required to automatically mail the Form 1095-C to individuals. The forms are available electronically on our website.
To access your 2025 form, follow the steps below:
- Log into your account on Workday
- Navigate to your employee profile
- Under your name in the left panel of your profile landing page, select "actions".
- Hover over benefits
- Select "view my ACA forms"
- You will then see a table containing a list of every plan year a 1095-C form was generated. Navigate to the year you need, and click “view/Print”.
You will have access to the electronic 2025 forms no later than January 31, 2025, and they will remain accessible to you through Workday for the duration of your active employment. You may request a paper copy of your 2025 form upon losing access to our website, such as when your employment ends. We may provide the copy to you electronically with your consent.
Requesting a Paper Copy
If you would like to receive a paper copy of Form 1095-C, you can request the form through one of the following methods. Your requested paper copy will be mailed US First Class as soon as administratively feasible, but no later than 30 days from the date of your request.
